When AI Gets It Wrong: How to Catch Errors Before They Harm Your Students

More than half of special ed teachers are now using AI to help write IEPs and support plans. That's not the problem. The problem is that most of them have never been told what to watch for when the AI is wrong and the stakes couldn't be higher.


Picture this: It is 9:30 on a Thursday night. You have eight IEPs due by the end of the week. You have already put in a full day of instruction, attended two meetings, fielded four parent calls, and handled a behavioral incident that nobody else had time to process with you. You are exhausted.


You open an AI tool. You paste in a student summary. Within seconds, you have a draft IEP goal. It looks professional. It sounds specific. It even mentions the student's grade level. Relieved, you drop it into the document and move on to the next student.


Researchers at CIDDL describe exactly this scenario in their February 2026 framework on ethical AI use in IEP development. They call him Mr. Schrute. He is a stand-in for every special educator working under impossible conditions, reaching for a tool that promises to make the impossible manageable. And the researchers are not here to shame him. They are here to warn him and you about what happens when that draft does not get the scrutiny it deserves.


Because when an AI gets an IEP wrong, the consequences do not show up in the tool. They show up in the classroom. In a child who does not make progress because the goal was never really theirs. In a family that loses trust because the document reads like it could have been written for anyone. In a district that faces legal exposure because the plan does not meet IDEA's individualization requirements.


This post is not an argument against using AI. Most special educators already are and for good reason. It is an argument for using it with your eyes open.


First, the Numbers You Should Know


The Center for Democracy and Technology surveyed 275 licensed special education teachers in mid-2025. Here is what they found:


  • More than half of special education teachers now report using AI in some way to help develop IEPs, a dramatic jump from the year before.

  • 15% said they use AI to write a full IEP or 504 plan, nearly double the rate from the previous year.

  • 31% use AI to identify trends in student progress and set goals. 30% use it to summarize IEPs. 28% use it to select accommodations.

  • Only 22% of teachers across all subjects said they had received any training or guidance on the risks of AI, like inaccuracy or bias in outputs.


Read that last number again. Nearly eight out of ten teachers are using AI tools with no formal guidance on what can go wrong. That gap between adoption and training is where students get hurt.


The Four Ways AI Gets It Wrong in Special Education


AI tools fail special education teachers in predictable, documented ways. Understanding these failure modes is the first step to catching them.


1. The Boilerplate Problem

AI models are built to generalize. They have been trained on massive amounts of text, and they are exceptionally good at producing language that sounds right. The problem is that sounding right and being right are two very different things in special education.


The CDT report warned explicitly that certain AI tools develop IEPs based on very little student-specific information and that using those IEPs without significant editing likely would not satisfy IDEA's individualization requirements. Researchers flagged a specific red flag: if more than 5% of your caseload's goals use identical or near-identical phrasing, that is a sign you are producing mass IEPs, not individualized ones.


A goal that reads "Student will improve reading fluency to 90 words per minute with 80% accuracy across three consecutive trials" might look perfectly SMART. But if that goal appears in seven of your eight students' IEPs this month, something has gone wrong. The AI has given you a template dressed up as an individualized plan.


What to watch for: Goals that could apply to any student with a similar disability category. Language you recognize from previous AI-generated drafts. Accommodations that do not connect to specific assessment data you provided.


2. The Bias Problem

This one is harder to see and more dangerous because of it.

The CDT report stated plainly that AI tools show racial bias. Students with disabilities are already underrepresented in the data sets used to train large language models. The result is that AI tools may produce goals, accommodation suggestions, and behavioral strategies that are calibrated to white, middle-class norms and that subtly misrepresent or underserve Black students, students from low-income families, and English language learners.


In a field already grappling with documented racial disparities Black students are disproportionately placed in restrictive settings and more likely to be identified with emotional disturbance rather than learning disabilities AI that reinforces those patterns is not a neutral tool. It is an amplifier.


What to watch for: Behavioral language that pathologizes cultural communication styles. Goals that assume access to resources (quiet home study space, parental reading support) that may not reflect a student's actual home context. Accommodation suggestions that do not account for a student's home language or cultural background.


3. The Privacy Problem

IEPs contain some of the most sensitive information about a child that exists anywhere: disability diagnoses, health histories, assessment scores, behavioral records, family circumstances. When you paste that information into a general-purpose AI chatbot to generate a goal or a progress note, you are almost certainly sending it to a server that will store it possibly indefinitely.


The CDT researchers were direct: the privacy risks vary depending on which tool you use and whether your district has a data-use agreement with the vendor. Most teachers using free or consumer versions of AI chatbots have no such agreement in place. Georgia, notably, is one of the only states that specifically names IEPs in its AI guidance, advising educators not to use AI for "high-stakes" purposes like IEP development.


What to watch for: Whether your district has an approved list of AI tools with vendor agreements. Whether you are entering a student's real name, diagnosis, or specific evaluation data into a prompt. Whether the tool you are using has a FERPA-compliant data policy.


4. The Legal Compliance Problem

General-purpose AI tools are not trained on IDEA, on your state's special education regulations, or on your district's policies. They do not know that your state requires specific transition language for students over 16. They do not know that your district uses a particular progress-monitoring system. They do not know that the accommodation they just suggested extended time on all assessments conflicts with the student's evaluation team's recommendation for chunked assignments instead.


Elevate K-12's analysis put the legal risk bluntly: if an algorithm generates a goal statement, who is legally responsible for its accuracy? The answer, under current law, is you. The IEP team. The district. Not the AI company. If a goal fails to meet IDEA's measurability requirements, or if an accommodation does not align with the student's present levels, or if a transition plan is missing required elements — those failures belong to the humans who signed off on the document.


What to watch for: Goals that are not measurable (no baseline, no criteria, no timeline). Missing required IEP components for your state. Transition language that does not reflect the student's age-appropriate transition assessment. Accommodation suggestions that contradict the evaluation data.


The Good News: AI Can Be Used Well


Here is the part of this conversation that sometimes gets lost in the alarm: the research on AI-generated IEP goals actually shows promise. A 2026 study published in the Journal of Special Education Technology comparing IEP goals written by experienced teachers with and without ChatGPT found no statistically significant difference in quality ratings. Teachers using AI with proper guidance produced goals of equal or higher quality while spending less time on drafting.

The operative phrase is with proper guidance. The research is not saying AI is a safe shortcut. It is saying AI is a capable collaborator when the human in the room knows what they are doing and stays in the driver's seat.


The CIDDL framework from February 2026 proposes a model that works: treat every AI-generated draft as a starting point, not a finished product. Share it with your IEP team at least 48 hours before the meeting so there is time for real review. Run it through an individualization checklist. Flag any goal phrasing that appears verbatim across more than a handful of your students. And if your district has approved purpose-built tools that integrate student data and include IDEA-aligned guardrails — use those instead of general consumer chatbots.


Your AI Review Checklist: Before You Finalize Any AI-Assisted IEP


Print this out. Put it next to your computer. Run every AI-generated draft through it before it goes into a student's official record.


☐  INDIVIDUALIZATION CHECK: Could this goal or accommodation apply to any student with a similar disability, or is it specific to this child's assessment data, strengths, and present levels? If it could apply to anyone, rewrite it.


☐  MEASURABILITY CHECK: Does every goal include a baseline, a target, a measurement method, and a timeline? Goals that cannot be measured cannot drive instruction and will not hold up in due process.


☐  BIAS CHECK: Does the language reflect this specific student's cultural background, home context, and communication style? Or does it assume resources and norms that may not apply to this child?


☐  PRIVACY CHECK: Did you enter any real student names, diagnoses, or sensitive data into the prompt? If so, are you using a district-approved tool with a FERPA-compliant data agreement?


☐  LEGAL COMPLIANCE CHECK: Does this plan include all required components under IDEA and your state's regulations? For students 16 and older, is transition planning present, complete, and grounded in age-appropriate assessment?


☐  TEAM REVIEW CHECK: Has the IEP team not just you reviewed this document before the meeting? Are general education teachers, related service providers, and the family able to see it in advance?


☐  AUTHORSHIP CHECK: Can you, as the teacher of record, stand behind every sentence in this document? If it went to mediation or a due-process hearing tomorrow, could you explain and defend each goal and accommodation?


What to Ask Your Administration Right Now


If your district does not yet have an AI policy for special education, you are not alone, but you should not wait for one to materialize before asking questions. Here is what to bring to your administrator, your special education director, or your union:


  • Which AI tools, if any, has the district approved for use with student data? Do those tools have FERPA-compliant data agreements?

  • Will the district provide professional development on the risks of AI in IEP development, not just training on how to use the tools?

  • Are there disclosure expectations? Should families be told when AI was used in drafting their child's IEP? Several researchers argue this should be standard practice.

  • Who is liable if an AI-generated IEP fails to meet IDEA requirements? The teacher? The district? Make sure that question has a clear answer before you are the one being asked it.


You Are Still the Expert in the Room

There is a version of the AI-in-special-education story that is genuinely hopeful. Special educators are drowning in paperwork while their students need them present, engaged, and thinking, not typing. If AI can draft a progress note in two minutes instead of twenty, or surface accommodation ideas that a teacher might not have considered, that is a real benefit. Research suggests it can even produce IEP goals of comparable quality to those written by experienced teachers when those teachers remain actively involved.


But the prerequisite for that hopeful story is the thing that no AI can provide: your professional judgment. Your knowledge of this specific child. Your memory of what his face looked like when he finally decoded that word, or how she communicates anxiety before it becomes a behavior, or what the family shared in last year's meeting that never made it into any formal assessment.


AI can draft. You decide. That distinction maintained with discipline, even at 9:30 on a Thursday night is the difference between a tool that serves your students and one that fails them.
